Changing information on your account can be as easy as logging into your online account , completing a form to send via mail or fax or picking up the telephone.

After logging into your online account, you can change your address of record.

Change Your Name

Requests to change a name must be made in writing.

To change a name on your account due to marriage, divorce or any similar reason, complete the Name Change Request Form and obtain the necessary stamp. Mail us the completed form using the address listed on the form.

Correct a Tax Identification Number

Requests to change or correct a tax identification number entirely must be made in writing.

Complete the appropriate Form W-9 from the IRS website and add reference your account number. Mail us the completed form or fax to 1-800-225-4240.

Add or Change Your Bank Information

Requests to add or change bank information must be made in writing.

Bank information can be submitted on the Account Application when establishing a new account. To add or change the bank information at a later date, complete the Account Options Form for your account type and obtain the necessary stamp. Mail us the completed form using the address listed on the form.
